EMPLOYMENT-Substitute Teachers

Thank you for your interest in becoming a substitute teacher for the Roland-Story Community School District!

Please see below for a list of required employment forms and any other information we may need. Click each item to access/download the form. Completed forms may be delivered to our Central Office, Monday-Friday, between 8am and 4 pm. We are located at the High School building (1009 Story Street, Story City, IA 50248), Entrance #1, across from the city swimming pool.


Statement of Health/Physical Form (Please have your physician complete this form--if you have had a physical exam within the last 12 months, we can accept proof of that exam in lieu of the form.)

Federal W-4 (For federal tax withholding)

State W-4 (For Iowa tax withholding)

USCIS Form I-9 (In addition to completing Section 1 of this form, you will need to provide us with a document or documents that validate your identity and work authorization. Please see page 3 of the form for a list of acceptable documents. You may provide one document from List A or one document from List B and one document from List C. Please bring the original document(s) to our office when you deliver your other forms--we must see the original ID document(s)-not a photocopy.)

Workers' Compensation Medical Treatment Acknowledgement

Additional information: Please provide a copy of your current teaching license or substitute authorization from the Iowa Board of Educational Examiners. If you have a teaching license, please provide a copy of your transcripts (we accept unofficial copies and photocopies).

On your application, please indicate if you have preferences or if you're willing to substitute at all buildings/grade levels. Once you submit your employment forms to our office, we will provide your name and contact information to the appropriate building principal(s). When a substitute is needed, a principal or their staff will reach out directly (we do not use an electronic or text message-based absence management software).

We currently pay $110.00 per day (pro-rated for partial days). Long-term assignments may pay more, depending on your education and placement on our teacher salary schedule.
